shannonshondeff@fit4mom.com to inform. The request must be received at least 5 days before next payment. If request is made 4 or fewer days before payment due we cannot guarantee your hold will go into effect before your next payment.
If on HOLD, memberships will be placed on hold for the maximum of 3 months. Our system will automatically take your account off hold on the date without any written notice, it is your responsibility to know the date your hold will be taken off. You may not cancel your membership while on hold. If you choose to CANCEL your membership while on hold, your membership will be reinstated for 30 days per cancellation policy. During this time you will have one more prorated payment until your membership is canceled.
If requesting a CANCEL, a 30 day notice (this form and an email stating to
shannonshondeff@fit4mom.com) is required. Last payment will be prorated based on 30 day notice.
