I, ______________________________, the parent or legal guardian of __________________, a minor, in my own behalf of the participant, grant permission to allow her/him to participate in the Wakefield High School Dance Team Warrior Clinic. I acknowledge that this activity may subject the participant to the possibility of physical illness or injury and assumer all risks associated with this event. I hereby give the athletic staff permission to apply first aid treatment for an injury sustained during the participation of the dance clinic until the parent/guardian can be contacted. In the case the parent/guardian cannot be reached, I give consent for the staff to use their own judgment in securing medical aid, ambulance service and necessary hospital admittance, when needed, as the result of injury during participation in the Wakefield High School Dance Team Warrior Clinic. Having read this waiver and understanding these facts, I, on behalf of the participant, waive and release Wakefield High School and any other individuals assisting at the clinic from all claims of liability of any kind arising from participation in the Wakefield High School Dance Team Warrior Clinic.
