The purpose of this form is to allow LPGA Amateur members looking for a team or needing to complete a team to share contact information with those looking to do the same. PLEASE NOTE that it is up to the individual members to contact each other about the team openings.  LPGA Amateurs Staff will not be involved in creating the teams.
