We are hoping to partially reopen Breakfast and After School Club in September but please bear in mind we will need to run this provision differently in order to follow government guidelines and reducing the mixing of children from different groups and therefore we will have to run a limited service.

This provision should only be used if you do not have any other alternative childcare.

Please note that this is a request form for the entire Autumn Term (September to December).  Please select days which you need the provision for.  These will then be the days that you child should attend every week until the end of term 2 - there is no option to change the days on a weekly basis.

There are limited spaces available for both Breakfast and After School Club.  We will write to you to confirm if your child has a space.  Following this all bookings should then be paid for in advance using ParentPay.

If children do not attend regularly or if sessions are not paid for in advance places will be withdrawn and offered to children on the waiting list.

In order to give us enough time to group the children and respond to you please complete the form by midday on Wednesday 15th July.  Any requests later than this date will go onto the waiting list.

If you have more than one child please complete it once for each child.
