This form must be filled out so that we know who is going to be involved in band and strings this year.  
4th graders - you may only choose strings.
5th and 6th graders - you may only choose one activity - band or strings.  You may also choose Chorus!  
For example, you may choose Band and Chorus.  You may choose Strings and Chorus.  You can also just choose Chorus alone.

If you are a student filling this out, and you're not sure about your answer, please just write "Not Sure".
