WAIVER AND RELEASE OF LIABILITY:

I hereby acknowledge that, by participating or volunteering on CityServe Day, on Saturday, October 5th, I will be exposed to a variety of risks associated with volunteering. Participation in these endeavors may involve certain activities, which could include, but are not limited to:  traveling by vehicle on interstates and city roads; entering homes and buildings; walking on or near roads; interacting with residents and others, and performing repair/improvement activities. I hereby assume all of the risks of participating or volunteering in CityServe Day and any/all activities associated with the event,  including, by way of example and not limitation:

Any and all injury, disability, death, or loss or damage to person or property associated with my presence at, or participation or volunteering in, CityServe Day, whether arising from the negligence of the Releasees or otherwise, to the fullest extent permitted by law; and

Any and all risks that may arise from negligence or carelessness on the part of the persons or entities being released.  Releasees include, but are not limited to, COSILoveYou and its subsidiaries, affiliates, event participants, sanctioning bodies, operators, vendors, sponsoring agencies, sponsors, advertisers, and owners and lessees of premises used in connection with the CityServe Day, and any of their respective employees, agents, officers, volunteers, directors, and others individuals affiliated with the Event.

On behalf of myself, my heirs, representatives, beneficiaries, and assigns, I hereby agree to hold harmless, defend, and indemnify each of the releasees from any loss, liability, claim, damage, judgment, legal fees, or costs arising from or incurred due to my participation in or otherwise in connection with the event, whether arising from the negligence of the Releasees or otherwise, to the fullest extent permitted by law.

FOR PARENTS/ GUARDIANS OF PARTICIPANTS OR VOLUNTEERS OF MINORITY AGE:

This is to certify that I, as a Parent/Guardian with legal responsibility for the minor participant(s), do consent and agree to his/her releases as provided, and, for myself, my child and our heirs, assigns, and next of kin, I release and agree to indemnify and hold harmless the releasees from any and all liabilities incident to my minor child's/children's involvement, participation or volunteering in the event as provided above, to the fullest extent permitted by law, as well as the use of my child/ward's name, image or likeness as provided above. Participation in the endeavors associated with CityServe Day may involve certain activities, which could include, but are not limited to:  traveling by vehicle on interstates and city roads; entering homes and buildings; walking on or near roads; interacting with residents and others, and performing repair and improvement activities.

CONSENT TO MEDICAL CARE:

In the event that I or an associated Minor Participant becomes injured or ill and I cannot physically consent to available medical care, I authorize COSILoveYou Staff or Representatives to arrange for any medical care they deem necessary or advisable. I agree to pay the costs of any such medical care arranged for myself or any Minor Participant and I hereby release and waive any claims or causes of action and agree not sue the Released Parties based on the negligent, wanton or willful acts of any medical provider who provides medical services to myself or a Minor Participant.

NOTICE OF FILMING AND PHOTOGRAPHY:  

By participating or volunteering in the CityServe Day, you consent to interview(s), photography, audio recording, and/or video recording of yourself and its/their release, publication, exhibition, reproduction, licensing, alteration, adaption, exploitation, or other use, in any format or medium now existing or hereafter created, and for any purpose, including but not limited to, promotional purposes, web casts, telecasts, advertising, publicity, and public relations by COSILoveYou and its affiliates, representatives, sponsors, and licensees. You also waive any right to inspect or approve any interview, photo, image, video, and/or audio recording of yourself, taken by the event or any person or entity designated to do so by the event.

If you do not wish to be featured in film or photography content associated with CityServe Day, you must contact COSILoveYou Staff prior to the event, so that they may accommodate your request. Please email info@cosiloveyou.com for all questions or inquiries.

You agree that you have been fully informed of your consent, waiver of liability, and release before entering the event.
