*  This form must be filled out prior to departure of the athletic event for your student athlete to travel home with someone other than a parent/guardian (full details are explained in the Athletic Handbook).  Even after the form is filled out, the adult responsible for taking your student athlete must still sign the coaches roster after the athletic contest.  The driver must be 21 years of age unless they are an immediate family member (brother, sister).
