Our school district is one of 28 Highly Impacted schools blessed to receive support from the U.S. Department of Education under Title VIII of the Elementary and Secondary Education Act (Impact Aid). These funds are for school districts that serve large numbers of military dependents or other federally connected children. To receive these funds, we need to have a completed survey form on each student so that federal students may be identified. Please complete one form for each of your student(s).  We are also taking this time to update your information in our student information system.  

The form is divided into four sections:
