Dear Grade 4 Parents/Guardians:

We are excited about the upcoming basketball season. Grade 4 Girls practices will be held on Tuesdays at lunch and Fridays after school till 4:15. Players must adhere to our school code of conduct to play on this team. We will be playing in a tournament at the Our Lady of Assumption Elementary School on April 21st during school hours.  

Please complete the following permission form.

Date of tournament:   April 21, 2020

Students will need to bring: school gym strip (mandatory), lunch/dinner, healthy snacks and water bottle, and money for concession.
   
Departure time: 9:00 am

End time: Approximately 2:30 pm at Assumption (we will then walk back to HLCS for dismissal)

Supervision:  Mrs. Berg, Mrs. Goetz and parent volunteers

Transportation provided by: Walking (need parents to walk with us)
