This form is to be filled out if the Veteran is bringing the said family members to Stand Down on the Delta 2023. ALL adults must bring ID/DL, and minors can have Student ID, ID/Dl, or birth record to prove identity. COVID 19 vaccinations are encouraged, but not mandatory.  There will be masks available for personal choice users, but again, not mandatory. There will be test kits on-site if anyone feels the need to test themselves.
