Notice is hereby given that the Annual General Membership Meeting of the Members of De La Salle University-Parents of University Students Organization (“DLSU-PUSO”) shall be held on Saturday, 24 September 2022, at 7::30 A.M. via Zoom, at which meeting the following matters shall be taken up:

  1. Oath taking of New Members (Parents of Freshmen for AY 2022-2023);
  2. Call to Order;
  3. Approval of the Minutes of the Previous Annual Membership Meeting held on November 6, 2021;
  4. Report of the President;
  5. Report of the Treasurer;
  6. Ratification of all Acts and Resolutions of the Outgoing Board of Trustees;
  7. Proposed Amendments to the DLSU-PUSO Articles of Incorporation and By-Laws;
  8. Election of Individual College’s Trustee/s;
  9. Other matters; and
  10. Adjournment.

 -- Retain my information throughout my entire stay in the organization with aor upon written cancellation, whichever is earlier.  I agree and understand that my information will be kept and archived for documentation purposes.  Access to my personal dataGoogle Form will be limited to the DLSU-PUSO executive committee members, board of trustees and officers, and the AGA registration committee;

 -- Parent personal information includes any information about my identity such as but not limited to my name, name/s of child/children and (his/her Student ID number), affiliation in the UNIVERSITY, and other information necessary for the basic administration and instruction;

 -- Allow video recording or post a photo where I am included in any way, be it my name, appearance, or any other form of identity that represents me, on all media platforms;

 -- Improper use of the information in this Google Form or any DLSU-PUSO database (online, offline, or printed) will be subject to appropriate actions under the Data Privacy Act of 2012 and the UNIVERSITY's Handbook;

 -- I understand and am assured that necessary precautions will be taken by the UNIVERSITY and DLSU-PUSO to protect my personal information.
