Meal Delivery Form for Hazel Green Family Schools
Parents,

Beginning Aug. 19, MCSS will provide meals Mon.-Fri. at every school during the 1st nine weeks. Families must pick up meals at the school where each of their students is enrolled.  Students will receive a lunch and a cold breakfast for the next morning.

Students will be charged for meals at their current eligibility rate.

Breakfast:
- Full pay all ages $1.75
- Reduced $0.30.
Lunch:  
- Full pay PK-8th grade $2.25
- Reduced all ages $0.40
- Full pay 9th-12th grade $2.50

To help with social distancing we are encouraging everyone to make payments through the PayPams system. The instructions for this can be found at https://www.mcssk12.org/Page/235.

If you would like for your student(s) to receive lunch and breakfast, but are unable to pick up the food due to lack of transportation or another hardship, please complete the following form for assistance.
