Please complete this form as permission for your child to participate in the Golf Centre, St Peter's Cambridge Holiday Programme. If you have more than one student attending, please complete a form for each student.

It should be realised that this event does contain some element of risk. This risk is countered by carefully structured and sequenced activities, and by supervising with suitably competent staff.  It is important that students follow any instructions given to them and follow the few rules imposed on them.  It should also be realised that because of the changeable and unpredictable nature of this event that the risk can never be reduced to zero.

I delegate my parental authority and responsibility to the coaches/instructors involved, and in the event of accident or illness, I authorise any medical assistance and treatment as necessary.

I understand that my child must bring any prescribed/required medication (Epipen, Ventolin and antihistamines etc) along with an action plan in a named zip lock bag and advise the coaches where this is kept in the event of an emergency.,
