Agreement to Participate. All activities involve certain inherent risks. Regardless of the care taken, it is impossible to ensure the safety of all participants. SSSC Junior squash program involves vigorous, cardiovascular activity requiring sustained game play, endurance, coordination and squash skill. While great care will be taken in conducting the program, we are unable to eliminate all risk from the activity.
All squash players are expected to follow these safety guidelines:
1. Wear proper footwear.
2. Be alert for unanticipated hazards on the court.
3. Do not crowd other players
4. Consume adequate liquids during your training.
5. Follow all instructions given by your coaches.
6. Wear protective eyewear at all times when on the court.
7. I agree to follow the preceding safely rules, all posted safety rules, and all rules common to the game of
squash and squash training exercises.

I attest, contract, understand and agree that I am to be legally bound to the contents of the following waiver:
In consideration of South Sound Squash Club, accepting me as a member/guest/participant, I hereby stipulate and agree to the following:

1) I, the undersigned, HEREBY ACKNOWLEDGE AND CONFIRM that I am fully aware of the dangers and risks involved in the use of the equipment, facilities and programs offered at the South Sound Squash. I do hereby absolve and release the South Sound Squash Club from any and all responsibility, or liability, and I agree to indemnify and hold harmless the South Sound Squash Club, its Directors, Officers, Coaches, its employees, or agents, from and against any and all actions, claims, costs, expenses, or demands of any kind whatsoever, or howsoever caused, whilst I am participating in any activity under the direction, supervision, or control of the South Sound Squash Club facilities, equipment or programs and I confirm that the use by me of said equipment, facilities, or programs is at my own sole risk.

2) That I am physically sound and that I have medical approval to proceed with a normal routine of exercise and that all exercise shall be undertaken by me at my own sole risk. That I am in good health and have no physical conditions that would be aggravated by my involvement in cardiovascular exercise, weight lifting, weight training, and/or functional training, nor do I have any physical limitations that would preclude said involvement. That I will communicate, verbally or otherwise, any health-related conditions that might affect my ability to safely take part in any SSSC Squash program.

3) That I will abide meticulously by each and every of the South Sound Squash Clubs rules and regulations. I further agree that if I derogate from abiding by the same, the South Sound Squash Club and its staff has the right at any time, whereby they in their sole and absolute discretion may see fit, to cancel any membership of the club or prohibit your participation in any program offered by the South Sound Squash Club.

4) That this release and waiver of rights shall be effective not only on the date hereof, but also on all occasions subsequent hereto when I shall use the South Sound Squash Club facilities, equipment or programs.

5) Image release: Images may be taken of the South Sound Squash Club members, guests or participants before, during and after programs or events. The Images are available for use on the SSSC website, our Social media handles and promotional material. That you accept these terms and give SSSC permission to use images for such purposes.

In consideration of being allowed to participate in any way in The South Sound Squash Club athletic sports program, related events and activities, the undersigned acknowledges, appreciates, and agrees that:

1. The risks of injury and illness (ex: communicable diseases such as MRSA, influenza, and COVID-19) from the activities involved in this program are significant, including the potential for permanent paralysis and death, and while particular rules, equipment, and personal discipline may reduce these risks, the risks of serious injury and illness do exist; and,

2. I KNOWINGLY AND FREELY ASSUME ALL SUCH RISKS, both known and unknown, EVEN IF ARISING FROM THE NEGLIGENCE OF THE RELEASEES or others, and assume full responsibility for my participation; and,

3. I willingly agree to comply with the stated and customary terms and conditions for participation. If, however, I observe any unusual significant hazard during my presence or participation, I will remove myself from participation and bring such to the attention of the nearest official immediately; and,

4. I, for myself and on behalf of my heirs, assigns, personal representatives and next of kin, HEREBY RELEASE AND HOLD HARMLESS the South Sound Squash Club their officers, officials, agents, and/or employees, other participants, sponsoring agencies, sponsors, advertisers, and if applicable, owners and lessors of premises used to conduct the event (“RELEASEES”), WITH RESPECT TO ANY AND ALL INJURY, ILLNESS, DISABILITY, DEATH, or loss or damage to person or property, WHETHER ARISING FROM THE NEGLIGENCE OF THE RELEASEES OR OTHERWISE, to the fullest extent permitted by law.

FOR PARTICIPANTS OF MINORITY AGE (UNDER AGE 18 AT THE TIME OF REGISTRATION)
This is to certify that I, as parent/guardian with legal responsibility for this participant, have read and explained the provisions in this waiver/release to my child/ward including the risks of the activity and his/her responsibilities for adhering to the rules and regulations. Furthermore, my child/ward understands and accepts these risks and responsibilities. I for myself, my spouse, and child/ward do consent and agree to his/her release provided above for all the Releases and myself, my spouse, and child/ward do release and agree to indemnify and hold harmless the Releases from any and all liabilities incident to my minor child’s/ward’s involvement or participation in these activities as provided above, EVEN IF ARISING FROM THEIR NEGLIGENCE, to the fullest extent permitted by law.

SOUTH SOUND SQUASH CLUB DATA PROTECTION POLICY
In order to fulfil its purpose and operate effectively the South sound Squash Club (SSSC) may collect, store and process personal information about members, volunteers, staff, service providers and suppliers. SSSC, as the Data Controller with responsibility for the management of personal data by its members and staff, recognizes the need to treat this data in an appropriate and lawful way and is committed to doing so.

By submitting this form:

I hereby acknowledge that the South Sound Squash Club (SSSC) is required to request and process the information provided within this registration form for the purposes of the individual/s membership of SSSC and give SSSC express permission to process this information as required in order to effect the orderly operation of SSSC in accordance with legislative requirements and those of CINSA (The Cayman Islands National Squash Association). Such processing to include, provision of details to CINSA for registration purposes, provision of details to our insurers, maintenance of up-to-date records including age, gender and medical information and conditions in relation to the members.
