This form is intended for you to enter your top 5 students (from each category listed below) from Round 1 of the 2021 Maths Championships. You will be asked to submit the top 5 students from the Year Level category/s that you teach.

Category 1: Year 7 & 8/Grades 6 & 7
Category 2: Year 9 & 10/Grades 8 & 9
Category 3: Year 11 & 12/Grades 10 & 11


This form must be submitted by Tuesday the 23rd of Feb.
