Instruction
1. Fill out this electronic form completely. For multiple certificates, please request individualy.
2. Before submitting the form, make sure that you have attached a copy of your valid ID. For current EAMC Employees, you may use your EAMC ID and any valid government-issued ID for former EAMC employees.
3. The Accounting Section will prepare and send your request via email. For current records covering 10 years and below, the certificate will be sent within 3 working days. For requests covering more than 10 years, the processing time is within 7 working days.
4. Please check your email if the certificate is already sent to you. Please acknowledge the email upon receipt.
5. Download and print the desired number of copies of the certificate.
