This form is for SRCHC staff interested in an identity-specific Follow Up Session for discussion of the SRCHC Equity Learning Modules. We recognize that it can sometimes feel safer and more productive to share space with other participants of similar identity. Because we're not sure how many people are interested in each identity group, we hope to start here by gathering information. Based on types of groups people select interest in, we'll aim to create facilitation teams with some facilitators with shared identity wherever possible.

Once we have enough interest we'll work with participants to set a theme and date for the session before Jan 31, 2022.

We'd ask that you please fill out the following form to the best of your ability. This information will remain with Challenge Accepted, who will use it to reach out to participants over email and work to plan identity-specific groups at the best possibles times.

Because these groups depend on getting enough participant interest, we can't promise we'll be able to run groups for every identity group or theme, but we'll make sure to connect with anyone interested to help find the best possible options. Please expect an email from Challenge Accepted in the weeks after submitting your form.
