We would be grateful if the parent or legal carer could complete this form to facilitate the attendance of your teenager under the age of 18.  Please confirm that you give consent for your teenage under the age of 18 to attend Coast 2 Coast meetings organised by Young Life Ireland in association with United Beach Missions by completing this form.
