預約流程：
因攝影機器材需由館員現場清點借用，請先填寫本申請單，經館員電話確認後才為預約成功。請於1個工作日前提出申請，恕不接受當日預約
--借用資格：需為本校(含附設醫院)現任教職員工生
--設備說明：含Sony NX5N硬體攝影機(含充電座與電池1顆，需另自備SD卡)、腳架、指向性麥克風及攝影機說明書一份
--借用時間：週一至週五9:00-16:30
--報到說明：預約時間30分鐘內，需至1樓 i服務台向館員報到，未報到則取消
--使用區域：拍攝區域限定為學習共享空間的預約房間，如簡報練習室、樂活室、會議室與討論室等，不包括開放討論區、陽光角落與其他樓層等非包廂空間
--其他：設備不可攜出館外，如有損壞需依維修費照價賠償，可參考「學習空間管理要點」第五條、設備損壞處理與賠償(

--停權：若預約後無事先通知取消或未領件者，同1人滿2次將暫停借用30天

Please request dates at least 1 working days from today’s date.
(1) Opening hours: Mon. to Fri. 9:00-16:30
(2) Using area: only in Conference Room, Group Study Room, LOHAS Space, Studio, Video Viewing Room.
(3) Suspension: If you do not receive the item without any reason after the appointment, the borrower will be suspended for 30 days when reaching twice.
(4) Responsibility for Loss or Damage: Borrower is responsible for any loss or damage to the Equipment from the time Borrower takes possession of it until it is returned to the possession of NTUML.
