2024 Stork Support Request Form
Please use this form to request support from Stork Support of Northern Colorado. All information will be kept confidential. The only requirements for our services is completing this form and your willingness to communicate with a member of our team respectfully.

We are headquartered in Windsor, CO and proudly serve Larimer and Weld Counties. We have streamlined our service offerings to be more kit-based so that we can be equitable and sustainable as we believe that every new family is worthy of a strong start through support.

You only need to complete this form once for the calendar year. You may request additional support throughout the year for diapering or infant feeding by just reaching back out to us using your preferred contact method.

When we have your requested items ready, will you be able to pick up at our home-base in Windsor (major crossroads are HWY 257 and Garden Dr)? If you do not have access to transportation we can coordinate a volunteer to deliver. Please note that delivery days and times will be dependent on our volunteers' availability and we cannot deliver outside of our service area. It may take up to two weeks to secure a delivery volunteer. *

Maternity Support

In clothing we accept gently used items and ensure they are clean and without stains or holes. We work to coordinate 8 seasonable outfits per box. Please select what would be most helpful to you:

Newborn Essentials

Please let us know what gender you would like your items. We do our best to ensure that all donations are working, clean, stain-free and without holes. All items and numbers of items in bundles and kits are as donations allow. We will always offer what we have available.

Hygiene Support

Don't have a newborn, but could use support with hygiene products? We are happy to make a custom hygiene kit for you as donations allow. Please let us know what would be most helpful:
Diaper Support

We are not a diaper bank, but are here to help in an emergency or to fill in the gaps of support from our local diaper banks within baby's first year. Please let us know what size you are needing.
First Year Feeding Support

We are here to help you have less stress feeding your baby within the first year of life with practical support. Please let us know what would be most helpful.
If your baby is formula fed, please let us know what brand/type and how many ounces per day baby is currently drinking (oz/bottle X bottles/day):
