AGREEMENT & SUBMISSION

I understand that to be considered for any position with Camps in Common, I must participate in a personal interview
by the camp director or a designated representative. I understand that, if hired, I will need to provide proof of
citizenship or authorization to work in the United States. I understand that successful completion of a pre-employment
physical may be a condition of employment.

I hereby authorize CIC/OFRC to contact any individuals and organizations identified in this application to verify

organizations to release to you any and all information that they have about me. I will not bring any claims against
Camps in Common or against any individual or organization based upon references provided. I further understand
that, if offered employment, such employment is subject to receipt of satisfactory references. I authorize Camps in
Common to perform a Background Check.

and non-compliance therewith may subject me to dismissal at the organization's option. I further understand that, if
