I am requesting authorization to borrow the technology device/equipment listed below for school and home use. Home use refers to any usage off-site of an Antelope Valley Union High School District (AVUHSD) owned, leased or rented facility. I certify that the equipment will be used only for appropriate school related activities and in accordance with the AVUHSD Technology Use Agreement (see below). I understand that I am not to upload/download on the equipment any personal materials that are inappropriate for school use.

I understand that I am responsible for any loss or damage to this equipment and any and all accessories (ie: power supply, protective case) until it is returned to AVUHSD. I will not loan or allow anyone to borrow the equipment referred to in this agreement. If the item is stolen, I will make a report with law enforcement and immediately provide a copy of the report to the AVUHSD staff. I will not make or authorize any repairs on the borrowed equipment. Should repairs be necessary, contact your school site to arrange for technical support, repairs, or replacement.

I understand that the equipment must be returned at the time my student is withdrawn or completes the program for which the equipment has been loaned.  I am taking possession of the equipment in good working condition.

Use of the technology provided by or used from the Antelope Valley Union High School District carries with it a serious responsibility. The use of the technology is intended to advance and promote educational opportunities for the students and staff of their respective school environments. Any person who uses the technology resources provided by the AVUHSD must do so within the careful guidelines set forth in your PowerSchool Enrollment account.

Access to technology resources are available to students who have this technology use agreement on file and use the resources in a responsible manner. Student use of technology resources on campus must be supervised to ensure that their activities are educationally appropriate. Staff members are required to be present and supervise students within the scope of their responsibility. Parent(s)/Guardian(s) are encouraged as well to supervise in the use of technology by their student(s).
The AVUHSD has taken precautions to deter access to inappropriate materials that exist on the Internet. However, on a global network, it is impossible to control all materials, and a user may discover inappropriate information. We do not condone the use of these materials or permit is in the school environment. The AVUHSD firmly believes that the valuable information and interaction available on this worldwide network far outweigh the possibility that users may encounter material that is inconsistent with the educational goals of the District. The cooperation of staff, students, and Parents/Guardians is needed to assure that educationally inappropriate sites are not used. Before students take advantage of the Internet access and the other District technological resources provided, it is required that each student receives appropriate orientation and instruction.
All stolen, lost or damaged technology will be reimbursed to the AVUHSD by the undersigned, and held liable for these monies owed.
   • I have read, and understand the policies and regulations explained to me in the Info PowerSchool Enrollment account.
   • I understand that the AVUHSD will provide access to technology resources and/or materials related to the school’s
      educational needs.
   • Internet access is a privilege, not a right and this access will be used only for educational purposes.
   • I understand that I do not have a right to privacy in the District’s technological resources, and that the District reserves the
     right to monitor any District technological resources for improper use.
   • The Parent/Guardian understands that close supervision is recommended at all times.
   • The student is aware that these rules apply whenever and wherever the student accesses this technology while using District
     funded resources.
   • Failure to abide by these obligations and responsibilities may subject student to disciplinary action, including suspension,
     revocation of the right to use technological resources, and possible legal action.
   • I agree to report any misuse of the information system to a District or school official.
