After submitting this application, please also EMAIL PROOF OF RESIDENCY (current electric, water, or gas bill, or property tax statement) TO VNAFZIGER@ARCHBOLDSCHOOLS.ORG.  

***THIS APPLICATION WILL NOT BE CONSIDERED COMPLETE UNTIL PROOF OF RESIDENCY HAS BEEN RECEIVED.***

***If you have guardianship, sole custody or shared parenting of this student and there is applicable custody court papers, they MUST be submitted with this application (email to vnafziger@archboldschools.org).  
Failure to provide this paperwork will cause delays and will result in a possible denial of application.
