We will need a Google Form submitted for each student you have enrolled in the Polo R-VII School District.

A paper copy of the Student Health History form will be sent home to be completed and must be returned to the school district.

Any online payments can be made through the Infinite Campus Parent Portal once you have completed the registration forms.
