Phase 2 Tester Application for SBY by Burns Well
Burns Well LLC is looking for 14 testers - 4 testers with interest in beard care, 8 testers interested in skin/body care, and 4 testers that are control group with zero usage in these products.  As compensation for this 30 day test, you will receive the product for testing free of charge, and after 30 day testing is complete will receive an eGift certificate for BurnsWellCo.com, conditions apply*. All tester applicants will be required to provide all allergen information, including prior allergic reactions to skincare products and current medication reactions and interactions, but are not required to state any name of medication they are currently prescribed. 
This testing date range is aimed for November 30th, 2022-January 9th, 2023. All testing must be completed within this range of time and all forms, questionnaires, and surveys completed prior to January 9th, 2023. 
Testers may be required to sign a Non-Disclosure Agreement governing the laws provided in their respective states. 
Submission of this form is not a guarantee of product testing status for Burns Well LLC. All testers chosen will be notified prior to November 15th, 2022.  All information provided by applicants in this form is confidential and will remain as such.
