NAEA- 2024 Exemplary Practices Cohort Form Application 
This form is designed to gather data and important information about your school's application to join the 2024-2025 Exemplary Practices Cohort.  As we evaluate the suitability of your school for the upcoming cohort, this form must be completed with FULL TRANSPARENCY regarding the strengths and potential areas of improvement for your school or program. The Exemplary Practices Cohort is designed to assist schools and educational leaders in fully executing the 15 main components of an effective non-traditional school. Application closes May 1, 2024.

Leadership - Please provide an overview of your school leadership team (e.g. number and title of administrative or leadership positions) and the role each of these individuals has within your school.  For example, is there a specific individual in charge of content areas, school climate and culture, scheduling, etc.?

Climate and Culture - How would you describe your school's climate and culture? Please include school data from the 2022-2023 school year.  This data should include a demographic breakdown of your student and staff population (e.g. ethnicity, gender, FARMS, ELL, SPED) Also include suspension and disproportionality data, promotion rates, graduation rates, and data related to students transitioning back to their comprehensive schools.

Staffing and PD- Please provide data pertaining to any surveys given to staff pertaining to job satisfaction and how they view professional development.  In addition, indicate the process you use to determine what professional development is needed for your staff and how you evaluate the effectiveness of PD.

Curriculum and Instruction - Please indicate what percentage of your staff are considered highly qualified in the content area they supervise or teach. Does your school utilize a universal instructional methodology (e.g. exploratory/expeditionary, differentiated instruction, project-based learning, direct instruction, etc.)? Are there opportunities within your school for adjustments to district/state issued curriculums?

Student Assessment - Please provide data pertaining to end-of-year assessments given to students for the 2022-2023 school year.  Also include matriculation data and graduation rates, based on both the 4-year and 5-year adjusted cohorts.  Adjusted cohorts refer to the number of students who graduated high school within a specified number of years of entering high school.

Transition Planning and Support - Please describe the process used for evaluating students entering your school to ensure student-specific supports are in place to ensure success and a possible transition back to your comprehensive school. Is there a process for ongoing evaluation of the services you provide to students?  If so, please describe this process.  If you use programs such as Check and Connect or provide specific services surrounding dropout prevention, please include this in your response.

Family Engagement - Please provide data from the 2022-2023 school year pertaining to efforts and feedback regarding family engagement.  Also include various programs or initiatives utilized by your school to promote open communication, dialogue, and information sharing.  How does your school currently utilize this feedback to drive program improvement?

Collaboration -  Please describe your process for including community businesses and outside agencies in your School Improvement Plan.  Is there a specific individual responsible for community engagement and the evaluation of this engagement toward the mission and vision of your school or school data outcomes? How is this individual evaluated in terms of performance and benefit to your building?

School Counseling - Based on the indicators listed in our Exemplary Practices, please describe how school counseling is utilized in your building.  Please include what data points or measures you use to evaluate your counseling program

School Social Work - Please describe how your school/program promotes educational equity, and removes barriers to learning is a characteristic of an exemplary nontraditional or alternative school. Please describe what data related to social work practices are collected and analyzed.

Digital and Virtual - Does your school/program utilize digital and/or virtual learning?  If so, please describe your process for selecting students and what supports you provide to promote academic and social-emotional success in this non-traditional format.  When considering unique needs that alternative settings are geared to address, what do you consider to be the biggest challenges regarding the implementation of digital and virtual learning and what steps are you currently taking to address these obstacles?

Policies and Procedures - Please describe the steps your school/program utilizes to communicate policies and procedures to your students, staff, and stakeholders. Personalized Education Plan - Does your school or program utilize PEPs for students (this is not to be confused with IEPs for students with disabilities)?  If so, please describe your process for determining who receives a PEP, which factors are taken into consideration for inclusion into the plan, and what stakeholders are part of its creation. If a PEP is utilized within your school/program, what role do students and staff take in its creation, implementation, review, and revision?  How often are these plans updated and what data is monitored to determine if the plan is no longer applicable or needed?
