A joint project of Breastfeeding Hawaiʻi and Mothers’ Milk Bank of San Jose

Breastfeeding Hawaiʻi has partnered with the Mothers’ Milk Bank of San Jose (MMBSJ) to launch the Hawaiʻi Human Milk Shipping Supply Site Project. Thank you for your commitment as a Human Milk Container Distribution Center (HMCDC). We greatly appreciate your time, efforts and support of breast/chestfeeding, and of human milk being shared with infants in need. Please let us know if there is any way that we can support you, and we always welcome feedback on improved resources for your area.

As a reminder, all Human Milk Container Distribution Centers (HMCDC) are required to report when they have provided a cooler (shipping container and supplies) to a verified donor. Coolers from MMBSJ are used solely for approved human milk donors, and are not to be used for any other purpose, including mom-to-mom milk sharing.

Please complete this report form whenever you have provided a cooler to a donor. You are welcome to complete this as you rotate coolers, or on a quarterly basis (due 3/31, 6/30, 9/30, 12/31).
