Please complete this form by December 5th if your child is signing up for Intramural Basketball. Basketball begins on Monday, January 6 and runs on Mondays and Wednesdays until 4:15. Parent pick-up in the front of the building is required. Students should dress accordingly, so they do not need to change at the end of the day.
