I understand that there is a $100 MINIMUM DEPOSIT DUE MAY 18TH (NON-REFUNDABLE). You will be emailed an invoice with instructions on how to submit your deposit after this form is submitted.
I understand the CJB Regular season fee is $950 (non-refundable) to Centerville Band Boosters and must be paid in full by Dessert Night in July (date TBD). Your $100 deposit will be credited towards the $950. Invoices with payment instructions will be emailed in July. Payment plans are available by request. I understand there is also a Pay-to-participate fee of $113 that will be billed with my school fees by Centerville High School once school starts in August.
