HOUSEHOLD INFORMATION

If you rent, you must provide us a written letter from your landlord or a copy of the section of your lease stating that you are allowed to have this type of animal and showing any limitations. If you live in a condo or townhome, you must show us written proof that you are allowed to have this type of animal there and show any restrictions.

TERMS AND CONDITIONS

AGREEMENT made and entered into this day of , between Arizona Pug Party and Rescue (hereinafter referred to as APPR), and (hereinafter referred to as Foster Parent, whether one or more); and, WHEREAS, APPR, in order to protect the animal, does accept for placement from individuals, animal shelters, humane societies, and other persons and entities, dogs which are unwanted, abandoned, neglected, and lost or mistreated; and,
WHEREAS, APPR does place such dogs in temporary, suitable, foster homes which can provide a normal family situation; NOW, THEREFORE, it is agreed as outlined below.

By checking the boxes below, you understand the terms of this form and the adoption of any of our animals. *

Foster Parent agrees to care for a rescued dog for a limited time. During this time Foster Parent will give kind and loving care to rescued dog and provide dog with a stable home environment.

Foster parent understands and agrees to adhere to all relevant zoning and animal codes and ordinances, whether local, county, or state.

Foster Parent understands that APPR has no prior knowledge of a dog's temperament, and will therefore, take all due care to protect himself or herself and others who may come in contact with the dog from bodily harm.

Foster Parent will not hold APPR responsible for damage to property or harm to persons caused by any fostered dog that is the property of APPR.

APPR will be responsible for any veterinary care charges incurred for said dog, but only if APPR has authorized such care, except in cases of extreme emergency.

Foster Parent understands that it may be necessary for him/her to drive rescued dog to veterinarian, training classes, displays and other places and is willing to do this.

Foster Parent understands that APPR will not be responsible for injury or illness contracted by any dog or dogs not belonging to APPR.

Foster Parent agrees that any dog fostered by him/her is owned solely by and is the property of APPR, and that Foster Parent will not remove any dog entrusted to him/her from the primary residence of Foster Parent or dispose of any dog in any manner without prior approval of APPR.

Foster Parent has the right to accept or refuse any particular dog.

Foster Parent will return dog to APPR upon request.

APPR agrees to hold Foster Parent harmless for accident or injury to any rescue dog in his/her care, except in the case of negligence on the part of the Foster Parent.
