I have read the following release and liability statement and agree.

I hereby release Poplar Ridge Friends Meeting and any and all adult sponsors or church staff from any liability in the event of an accident in route, during, and returning from the sponsored event. Should emergency medical treatment be necessary, I authorize Tim Vestal or any other adult sponsor of the event to act on my behalf and approve appropriate treatment. I further grant permission for any photographs or videos taken at this event of my child(ren) to be used for any lawful purposes in connection with the promotion and activities of Poplar Ridge Friends Meeting.

My child and I have read and agree to the following guidelines.

I will be sure to follow all guidelines for this event, realizing that failure to do so will result in disciplinary action, including the possibility of being sent home at my own expense.

Permission slips for all events and Medical Forms (one per year) for all overnighters are REQUIRED. Extras are beside Tim's office door and in the youth room.

I will be nice: respecting other participants and leaders and their property. I will take care of the facilities and property we are using, including helping clean up during and after the event. I will help keep trash out of the van! Get along with everyone or you ruin everyone's trip!

If damage is done to any property during one of our events (unless it is from an accident), the person(s) causing the damage will be expected to pay for any repairs or replacement.

I will be in my own room/cabin/tent by lights out and will never be in the sleeping areas of the opposite sex for any reason.

I will not be alone with a member of the opposite sex during the event and if I am in a relationship with someone else in the group, I will save my affection for after the event.

I will be at all scheduled activities unless I have made arrangements in advance with Tim Vestal. I will not drive any vehicle during the event unless arranged with Tim Vestal.

I will be off my cell phone/internet for personal reasons during any scheduled activity.

I will be sure to portray this event, myself, and others in positive, good taste, and Christian character in all social media related to this event.

I will be careful to make sure that what I wear during youth events portrays modesty. This includes what is printed on my clothing. During events that involve swimming, girls are expected to wear a modest, one-piece swimsuit.

I will not be in possession or use any tobacco products, e-cigarettes, vaporizers (etc.), non-prescription drugs, alcohol, knives, firearms, fireworks, or lighters.

I realize that photos and videos of my child and their name may be used in promoting this or other events.
