Questionnaire on Teacher Assistants/Instructional Aides/Math & ELA Intervention
Hello and thank you for considering taking part in this study on the effectiveness of support in and out of classrooms!

This study is being conducted as part of California State University, Long Beach's SOC 355: Quantitative Research Methods class.  The lead researcher is Aaron Galan, who can be contacted at Aaron.galan01@student.csulb.edu.

The purpose of this study is to examine the effectiveness of support in and out of classrooms for CSULB's SOC 355 class.  The survey will be used as a template and example for students creating their own surveys for their original research projects.  The data collected from this survey will be also used as an example for students to set up and analyze datasets using SPSS.  

This survey questionnaire will consist of 10 questions. There is no time limit associated with participation in this study.  Participation in this study is 100% voluntary and potential respondents need not feel any pressure to complete the survey and they can choose not to participate at all.  No identifying information will be presented in any report generated from this data.  Only aggregated data in the form of basic descriptive and inferential statistics will be made available in such a report, and individual responses will not be able to be traced back to any identifying information.  

The benefits of participating in this study will be to familiarize ourselves with teacher opinion on how effective support is.  Additionally, the example of the survey and the data generated by the survey may prove to be useful in the future as a template for student surveys or to generate and analyze statistical data.  

There are no specific identifiable harms associated with participating in this survey.  However, if there are any concerns about potential physical, emotional, psychological, social, financial, or legal harms stemming from one's participation in this study, please feel free to withdraw from the study or skip any specific questions you feel uncomfortable with.  Furthermore, if you have any questions, comments, or concerns about the nature of any of the survey questions please do not hesitate to contact the lead researcher at Aaron.galan01@csulb.edu.  

Participants are able to withdraw from this study at any time, for any reason without penalty.  No one is obligated to complete the survey, even after it has begun.
