I,  hereby, give my consent for the above named participant to enter the First Baptist Church Vacation Bible School.  I also, hereby, give my consent for any emergency/medical treatment approved by his/her teacher or other adult in case of illness or injury while participating in VBS.  I understand t his is to prevent undue delay and assure prompt treatment and that only a licensed physician will be engaged for such an emergency.  By entering your name you are legally agreeing to this statement.
