I understand that HOLY ROSARY Parish, the Catholic Diocese of Nashville, and Totus Tuus assume no responsibility for accidents which may occur in association with diocesan/parish events and activities. I agree to use my/our personal insurance to cover any such incidents. I understand that, in the event medical intervention is needed, every attempt will be made to contact the persons listed above. In the event those individuals cannot be reached, I/We hereby give permission to the physician or any other qualified medical staff selected by the event leader to hospitalize, secure medical treatment, and/or order injection, anesthesia, or surgery for Participant as deemed necessary. *
I hereby authorize the Catholic Diocese of Nashville and Totus Tuus and its agents to utilize photographed and/or video images of me or my child by my parish or the Catholic Diocese of Nashville. I further grant permission for the resulting photographs and/or videotaped footage to be edited, if necessary, and then published and/or broadcast for the purpose of promoting Church or Diocesan events and/or programs. I understand that I will receive no compensation should any photograph and/or video of me or my child be used. *
I understand I will have to sign a MEDICAL AUTHORIZATION form and a PHOTO RELEASE form on the first day of Totus Tuus *
I realize I MUST provide a LUNCH for the child (day program only) each day BUT the child will receive a snack and drink provided by the church each day. *
Would anyone in the family be willing to VOLUNTEER during the day program (high school or adults) OR Would you be willing to DONATE lunch or dinner for the TEAM one of the days of Totus Tuus?
