PLEASE READ THE FOLLOWING INFORMATION BEFORE SUBMITTING THIS FORM.

We are now accepting applications for the 2021-2022 school year. We follow the Gaston County School system cut-off date of August 31 when placing students in each class.

There is a non-refundable registration fee of $85 due at the time of registration. After you submit your form, you will receive an invoice by email that you may pay online via free bank draft, cash, or check. This payment holds your child's place.

Monthly tuition is due on the 1st of each month and considered late after the 10th. Late tuition is subject to a $20 late fee. Class begins on Tuesday September 7th, 2021. Our hours 8:30 am - 12:00 pm each day. We offer early drop off beginning at 8:05 am on a limited basis for an additional $25 per month.

An immunization record is required by the first day of school. All applicants to the 4 year old program MUST be toilet trained.
Orientation for will be held on Thursday September 2nd, 2021at 6:00 pm in the Sanctuary.

Monthly Tuition Rates:
2 days a week - $180
3 days a week - $190
4 days a week - $205
5 days a week - $220

Upon receipt of this completed form you will receive an invoice that may be paid via bank draft, cash, or check. Please make checks payable to FUMC Preschool. Additionally, you will be added to a mailing list to receive all communications regarding the upcoming school year.

I understand that my child will be placed in the appropriate class according to their age on August 31st as dictated by Gaston County Schools policies. I understand that 4 year old's must be fully toilet trained. I understand that if my 3 year old is not fully toilet trained there will be an added $10 fee per month and that this fee will be lifted when the child is fully toilet trained.
