1) Only a coach or adult chaperone should complete this form. If the complaint or allegation is about the school's own coach or adult chaperone, then a student should direct their concerns to their parent or guardians and/or the school's administration.  Please complete this form to make the tournament aware of equity concerns about competitors, judges, or coaches in the tournament. Please give complete detailed description of concerns.
2) The Tournament Directors will follow up with the person submitting the complaint and will make a plan to hear out the case.
3)The Tournament Directors will announce decision and possible implications to relevant parties only as necessary.

IMPORTANT NOTES:
1) This form cannot be used to reverse an already submitted decision in a debate.
2) We can not receive anonymous complaints since we need to follow up with the person who submitted the complaint.
3)We will keep the information disclosed as confidential as possible. Please be aware: ethically, tournament directors are required to take appropriate action and notify authorities if students engage in behavior that presents clear and imminent danger to themselves or others, in the case of known or suspected child abuse or neglect, or in the case of known or suspected sexual violence. Tournament directors are mandated reporters.
4)  Your report may result in multiple people being contacted to follow-up with your concern, including, but not limited to, school coaches, principals, headmasters, participants, and parents as is necessary.
