Agreement and Consent
Even though this is just an inquiry, I just need all the legal stuff given to you so we are all on the same page. Please type your initials at the bottom once you have read and agreed to the following:
1- Cake demonstrations are for entertainment factors only. Though you will leave with an abundant amount of knowledge and skills after each Event, this is not an accredited course by any Institution or State department
2- Cakes and frostings are outsourced commercially. Ingredients list can be provided. If anything is prepared in my home ahead of time, an ingredients list will also be provided according to the Utah Food and Agriculture Department guidelines.
3- I have a Food Handlers Permit, operating as a sole proprietor (Muyly Miller). As such, all liability, damages, complaints, publicly or privately, pertaining to supplies, ingredients, use of supplies, set up and break down etc., is released by the Host (you) including Venues and/or Homes, their owners, occupants, and/or employees.
4- Supplies are provided for each guest for use only. They are not for sale, a supplies list can be made available upon request
5- Alcohol is not provided or included in any of my demonstrations, nor am I responsible for minors consuming alcohol in my presence during my demonstration.
6- For food safety handling, a separate demonstration area is recommended, away from other food items that don't belong as part of the cake demonstration and each cake being decorated by guests.
7- Payment by individuals must be made in full 5 days in advance.
8- Lump Payment arrangements are 1/2 total due 5 days in advance, and remaining due the day of Event
9- Cancellation will happen only if the minimum 6 persons is not met and paid for 5 days before the Event
10- City and County curfew laws are generally around 10:00 pm. Please arrange your Event accordingly if the Cake demonstration runs past curfew. Being indoors usually helps, but in some instances, outdoor Events, or those held in multipurpose locations, can be bothersome to neighbors or general public if the noise levels are higher than normal (meaning: this is a heck of a class!}
11- If you are scheduling this as a third party, all parties involved will need to read and agree to this Agreement and Consent before any further plans can be arranged and discussed.
12- For promotional and advertising purposes, each Event may be photographed and published at some point whether live or at a future date.
14- In some instances, I may bring an assistant or another instructor with me, depending on party size- liability and damages discussed in line item 3 (three) will apply.
