Peer Leader Inquiry/Referral Form

All new, incoming students to the University are matched with a Peer Leader, who is an upper-year student mentor from their respective program and faculty. Upper-year students who think they would benefit from a Peer Leader are also welcome to 'opt-in' to the program.

Complete this referral form if:

a) You are a student in your first year at the University and you do not know who your assigned Peer Leader is
b) You are an upper-year student who would like to opt-in to the program and be matched with a Peer Leader
c) You are a staff or faculty member who would like to refer a student to the program to be matched with a Peer Leader
d) You are a student who has already been assigned a Peer Leader, but would like to be re-assigned to a new Peer Leader
