Application form for the Scholarship for the 2nd NCD Youth Conference
Dear applicant,

the International Youth Health Organization (YHO) is happy to announce the 2nd NCD Youth Conference.

The NCD Conference aims to bring together the Public Health and Youth sectors to ensure better working and living conditions of young people in Europe. Primarily, we would like to empower youth workers and leaders to set up a healthy environment for their constituents locally. At the conference, we will address the health literacy aspect of the context of NCDs.
The idea of the conference is to bring together from 80 to 100 public health professionals, youth workers, youth leaders and young people from across Europe, who are interested in increasing health literacy in the context of NCDs.

Please fill the form to apply as a participant to the 2nd NCD Youth Conference that will happen between 29th and 2nd September 2022 in Ljubljana, Slovenia (28th August - 2nd September 2022 including travel days).

BASIC PROVISIONS

Data collection, storage and protection:

For the purpose of organizing the event, YHO needs the following information of each participant of the conference: name, surname, e-mail address, date and year of birth, gender and your home country.

Each participant will have the opportunity to obtain a certificate for their participation in the conference. For the issuing of the certificate, we need your first and last name and your e-mail address. To receive a certificate, you can tag it below in the application form and we will send it to you by e-mail.

The above mentioned data (except for gender and birth date) and bank account details, obtained through your registration fee transfers, is obliged to be kept by YHO for ten years. Data will be stored and protected in accordance with the General Data Protection Regulation and the Data Protection Act, and only authorized persons will have access to them. All data for which YHO is not legally obliged to keep for 10 years, will be deleted after all procedures related to the event are finalised, except in the case that the participant have explicit permission to store and use his data.

DISCLAIMER

In YHO we have a “no-conflict of interest” rule, which means that any participant or organization, which is affiliated or has received funding from alcohol or tobacco industries or their intermediates, is ineligible for participation at this event. If found out on a later stage that the participant was falsely confirming the “no-conflict of interest” clause in the application form, the individual would be denied access to the Conference and the already paid participation fee will not be refunded. An application for membership does not mean a definite fee reduction – refund happens after potential approval of the new organization as a member by the YHO Board. The organizers will reimburse the participants for their expenses up to the agreed amount in no later than 90 days after receiving of all necessary documentation (all invoices that confirm the costs, original boarding passes and other tickets). The participation fee, after being transferred, will not be refunded unless a force majeure. Making the reservation of the accommodation on your behalf does not mean the organizer will cover any of the above-mentioned costs
