#FixTheForm Grantmaker Self Assessment
Thank you for taking this self assessment to gauge your foundation's degree of grant form fixedness.

Each question in this assessment is a pain point identified in 2,500 reviews on GrantAdvisor.org AND ranked by 500 grantseekers from nine countries in the #FixTheForm survey.

The questions in the assessment appear in the order of most painful to least, according to #FixTheForm survey findings. Each question is "select all that apply" so if there is additional context that you'd like to share, you can do so by also clicking "other" and explaining.

Your responses will be confidential, and no identifying information about you or your foundation will be made public.

1. Grantseekers can view and download our full application ahead of time. For example, our foundation makes a Word document version of our application available on our public website.

2. We know on average the amount of time it takes for grantseekers to apply to our foundation, and the cost of that time is commensurate with the amount of funding we award.
1 point
3. Grantseekers can save the information they enter and return  later to continue entering responses in our grant applications and report forms.

4. Our forms, taken as a whole, don't contain questions that are repetitive, or inspire responses that are repetitive.
1 point
5. If our forms have word/character limits, we provide adequate space for grantseekers to respond to the question we've asked. (Grantseekers report that working against technical limits of a field set too low adds hours onto the grant application process)
1 point
6. We only collect information in a format that is necessary for us to review it. For example, we don't require grantseekers to manually type out each board member's information in individual fields when we review their board list as a whole.
1 point
7. We accept grantseekers' templates for documents and information that we require/request (ie. board lists, budgets, etc.). (Grantseekers consistently report it can take 2-3 hours to translate their budget into a funder's template).
1 point
8. We provide our application in a format that is easy to edit and share. (Grantseekers report that a Word or Google docs is preferred over pdf)
1 point
9. Our eligibility requirements are clear; we consistently receive applications from grantseekers whose work is aligned with our giving priorities.
1 point
10. Our online application is able to save formatting such as bullet points or underlining.
1 point
11. (If your foundation provides a downloadable application), the questions in that document exactly match the questions found in the online system.
1 point
12. Our foundation does not require grantseekers to submit information that is already publicly available; our foundation allows grantseekers to import information from past requests or websites like GuideStar.
1 point
13. If our application includes multiple choice options, we provide an "other" option where grantseekers have the opportunity to explain or self describe.
1 point
14. Our foundation and/or grants management software provider offers contact information for technical support if grantseekers encounter problems with the online application/system.
1 point
15. Our foundation makes our application available to the public, and grantseekers don't need to create an account or register in order to view/download our application in advance.
1 point
16. Our foundation explains giving priorities and application questions using commonly understood words and phrases, avoiding special words or jargon.
