SMU Artdicted Asset Loan Request Form
Hello! This is the asset loan request form for SMU Artdicted materials.

Please make one submission per line item.

Submission of this form is not equivalent to the approval of the request. You will be contacted after submission. Inappropriate submissions will be ignored, and repeat violations will result in a subsequent indefinite ban from loan requests.

Rental rates:

Easel Stands
$2 per day (Weekdays)
$4 per day (Weekends)
If the loaned asset is damaged/lost, it is $10 per piece.

Brushes (Bundle of 10)/Palettes/Water Buckets

Brushes - $1 per day
Palettes/Buckets - $2 per day
If the loaned asset is damaged/lost, it is $2 per piece.

*Only Paynow/Paylah is accepted

***For loans that are overnight or end at a very late timing, please ensure you have a space to keep the items before returning them to SMU Artdicted at our convenience. If the assets are lost, you will be liable as well.

By submitting this form, you agree to the following conditions.
1. You (or your Club/CBd) will undertake all responsibility related to the upkeep and maintenance of item(s) loaned to you.
2. You will be responsible for the repair or replacement (where appropriate and necessary) of the loaned item(s) should there be any damage to or loss of the item(s) during the period of the loan, as determined by the SMU Artdicted Operations Director.
3. The SMU Artdicted Operations Director reserves the right to amend, without prior notice, the period of the loan of item(s) loaned to you, exercised at their own discretion.
4. For loans that are overnight or ends at a very late timing, please ensure you have a space to keep the items before returning them to SMU Artdicted at our convenience. If the assets are lost, you will be liable as well.

*Note that you will be subjected to pay the penalty of $10 per easel if the easel is found to be damaged in any of the following ways (but not limited to):

-Missing screws
-Broken wooden panels
-Vandalised Easels

*Note that you will be subjected to pay the penalty of $2 per item (Brushes/Palettes/Water Buckets) if the asset is found to be damaged in any of the following ways (but not limited to):

- Cracks on Palettes/Water Buckets
- Broken or Hardened Brushes
