Thank you for your interest in a Multnomah County Adult Care Home Program (ACHP) qualifying test. Complete this form to register for one of the ACHP-required tests on the date listed below. A confirmation email will be sent after you complete this form.
You must pre-register.
Please do not come to the ACHP office if you are sick or have symptoms of COVID-19 or respiratory illness such as fever, cough or shortness of breath.
Qualifying Test Date: Tuesday, May 14, 2024
Time: Sign-in begins at 1:45 p.m., testing begins by 2 p.m.
Location: Multnomah County East building, 600 NE 8th St, Gresham, OR 97030
Check in with Adult Care Home Program at Window 3 in Suite 100.
Government ID: You must have valid, current ID with you to take the test.
Two tests are offered at this time:- Developmental Disabilities Qualifying Test. You must bring your certificate of completion from the Self-Study Manual in order to take this test.
- Aging & People with Disabilities and Mental Health & Addictions Qualifying Test. You must bring your EQC Certificate(s) in order to take this test. Operator Applicants must complete EQC Essentials and EQC Administration. Resident Manager or Shift Manager Applicants must complete EQC Essentials. Please complete the ACHP Orientation training prior to taking this test.
You will only have time to complete one test. If you need to take more than one qualifying test, please select another date to register for additional tests.
