Charlie’s Angels Animal Sanctuary, Inc. Waiver

I hereby certify that I am over eighteen (18) years of age, and am of sound mind at the time of the execution of this Waiver, Release, Hold Harmless, and Indemnification Agreement (hereinafter referred to as “Release”) and agree to the following: I agree to release, hold harmless and indemnify Charlie’s Angels Animal Sanctuary, Inc. (hereinafter referred to as “Charlie’s”) from and against any and all loss, liability, claims, demands, damages or costs to myself or my legal minor if in my presence, or property suffered or sustained as a direct or indirect result of the participation at Charlie’s.

I, on behalf of myself and the minor(s) under my legal care or others in my party, acknowledge that playing with Charlie’s cats may not be supervised and we will be at our own free-will to engage with said animals. We assume the risk associated with this type of arrangement.

Charlie’s staff will remain on the premises to monitor the cat lounge and café area, offer support and encouragement, and be available to assist me if I need any assistance with or without the cats. I take on all assumed risk for myself and the legal minor(s) under my care or those in my booked party. I am aware of and fully understand the dangers involved in playing with and engaging with cats.

I acknowledge that I am voluntarily participating in such activities, or allowing the minor(s) under my legal care to do so, and these activities will not be covered under the insurance policy of Charlie’s Angels Animal Sanctuary, Inc. As a participant, I promise to watch the cats carefully and will not approach, or allow the minor(s) under my legal care to approach any cat that seems irritated, aggressive or whom is eating. Like most animals, cats can get aggressive in nature due to hunger, fatigue or stress and Charlie’s cannot be held responsible for injuries or damages that I, or the legal minor(s) in my care, may sustain while engaging the cats in any way.  I will use my best judgment to be aware of the cats and my surroundings, for myself and the legal minor(s) under my care. I will not pick up the cats and will refrain from loud or rambunctious movements and sounds so as to not frighten the cats.

I agree that myself AND the legal minor(s) under my care and those in my party, have no physical or emotional issue which would adversely affect my ability to play with the cats in a safe and appropriate manner. Furthermore, since minors are not of the mature age to make adult decisions and take adult actions, I, on behalf of myself and the legal minor(s) in my presence while visiting (OR for the legal minor under my care, who is allowed on the Charlie’s premises without my presence – aged 13-17 years old) agree to abide by all rules set forth at Charlie’s. IF I AM UNWILLING TO FOLLOW THE RULES SET FORTH, I AM FULLY AWARE THAT WE MAY BE ASKED TO IMMEDIATELY LEAVE THE PREMISES WITHOUT A REFUND. While on the Charlie’s premises, photographs and video may be taken for social media purposes. Your image may be used for this reason. If you choose NOT to have your photo taken while participating in Charlie’s activities, please let a staff member know.

I HEREBY EXPRESSLY RECOGNIZE AND ASSUME ALL RISKS ASSOCIATED WITH MY PARTICIPATION IN THE CAT LOUNGE AND VOLUNTARILY RELEASE, WAIVE, DISCHARGE, COVENANT NOT TO SUE AND HOLD HARMLESS CHARLIE’S ANGELS ANIMAL SANCTUARY, INC. I agree to indemnify and hold harmless Charlie’s Angels Animal Sanctuary, Inc. from any and all claims, actions, suits, loss, liability, damage or costs, including court costs and attorneys’ fees, that may be incurred due to my visit to Charlie’s.
