The Hazelwood PTO welcomes you to the 2020-2021 school year! Please take a few minutes to complete the following form regarding communication, Husky t-shirt size, and Class Parent interest. PLEASE submit ONE form PER CHILD so that we have a complete database of all students.

The Hazelwood PTO (Parent Teacher Organization) would like to ensure direct communication with all of our families throughout the school year.  We will be sending very little home via hard copy and want to make sure we have your information to communicate electronically.  This is our second year collecting information through Google Forms, so we ask that you complete the questions to create a database for the 2020-2021 school year.  All of the following information is used for school purposes only and will not be shared with a third-party.  Your privacy is important to us and will be kept private.

Weekly Newsletters - The Hazelwood PTO emails a newsletter each week that contains important school/PTO dates/information, upcoming events, event recaps, links to fundraisers, and much more.  Staying informed on school news is very important to help the school year run smoothly.  Please provide us with a working email that you check regularly.  You can expect newsletters from hwoodhuskies@gmail.com each Friday.

Class Lists - Each parent/guardian will be emailed a copy of their child's class list.  It will have student names, parent/guardian names, phone numbers and email addresses.  These class lists can be used to contact a classmate with homework/project questions, playdate/party invitations, and help Class Parents communicate with families.
