Remote Learner Monday Meal Pick Up
PLEASE COMPLETE THIS FORM IF YOU PLAN TO PARTICIPATE IN THE SCHOOL BREAKFAST & LUNCH PROGRAM.  PLEASE SUBMIT YOUR REQUEST BY WEDNESDAY THE WEEK PRIOR TO MONDAY PICK UP.  

FREE MEALS ARE AVAILABLE ONLY TO A ENROLLED CENTRAL GREENE STUDENT WITH A STUDENT ID.  YOU MUST PROVIDE A NAME WITH STUDENT ID TO RECEIVE MEALS.  

MEALS WILL BE BUNDLED (5 BREAKFAST & 5 LUNCHES) PER STUDENT.  NO WEEKEND OR HOLIDAY MEALS WILL BE INCLUDED.

MEAL PICK UP WILL TAKE PLACE ON MONDAY BETWEEN 9AM-10AM.  YOU WILL NEED TO GO TO THE FRONT OFFICE TO RECEIVE YOUR BUNDLED MEALS FOR THE WEEK.  IF THERE IS NO SCHOOL ON A MONDAY THE FOLLOWING DAY THERE IS SCHOOL YOU WILL BE ABLE TO PICK UP YOUR MEALS.

YOU MUST SPECIFY WHAT BUILDING YOU WILL BE PICKING UP AT.  YOU MAY CONTACT THAT BUILDING IF YOU HAVE ANY QUESTIONS OR CHANGES.
