(Obsolete 12/29/2023) Self Evaluation: Lead Registered Dental Assistant
This Google Form is to be submitted by the Lead Registered Dental Assistant who is due for their Performance Review (you) prior to your In-Person Performance Review. Your Lead Dentist will also complete an employee evaluation form. During the In-Person Performance Review, you and the Lead Dentist will discuss where your evaluations align and where adjustments or further development would be beneficial. 
You will be evaluating yourself on: 
  • Your exhibition of Forest Family Dentistry's Core Values; 
  • Your Lead RDA Technical Skills; and 
  • Your Lead RDA Soft Skills. 
Additionally, you will share the professional and personal goals you want to achieve over the next year. At the end of your In-Person Performance Review, you and the Lead Dentist will agree upon expectations for the following year to help you achieve your goals.
