As a parent/guardian, it is your voluntary decision to permit your student’s involvement in 2020-2021 IESA extra-curricular activities. Hollis Consolidated School District 328 will implement management and control measures in accordance with relevant CDC, IDPH, and IESA guidance for extra-curricular activities, but risks cannot be fully eliminated. By clicking below, I acknowledge that there are certain risks associated with my student participating in Hollis Consolidated School 328 activities and agree that my student may participate in team activities during the 2020-2021 IESA season.
