As a representative of the USA Lacrosse Women's Game for the 2022 National Tournament, I agree to the following:

- I will not use my position as a coach to benefit myself or a family member personally/financially in any way (i.e. giving more playing time to a player I (they) work with/coach).
- I will not use my position as a coach to promote any business (through literature, websites, or advertisements) or to attract players to play on a team I coach or camp that I run.
- I will keep all information as extremely confidential and not to be discussed with anyone outside of the sub-regional chair or USA Lacrosse staff.
- If selected to coach, I understand that I will/may have to obtain a Background Check through USA Lacrosse's third-party vendor, NCSI (free process).
