Greetings and welcome to the 2023-2024 school year. This form is for notification of absences, late arrivals, and early self checkouts. Completing this form does not automatically excuse absences. The determination will be made by the Attendance Officer in accordance with the City Schools of Decatur attendance policy (page 33). All requests must be submitted by the parent/guardian as documented in Infinite Campus. Students are not allowed at any time to complete this form for themselves, those entries will not be accepted.

If you are coming to pick up your child, you should not fill out this form. When you arrive to pick up your student, you will come to the doors leading to the attendance office off of N. McDonough, ring the bell and be prepared to show your ID to the office representative or a designee.

We understand there are unforeseen circumstances however, all early dismissals end at 3pm sharp. If an extension is needed please contact the attendance office at 404-370-4170 x2148.
