Welcome, families with children and youth! We’re glad that you’re joining us this year!

Please complete this form to register your child(ren) or youth into NSUC's Religious Education programs. All children from nursery through high school youth group must be registered. The information you provide helps us create an engaging and supportive faith formation experience in a loving and safe environment.

Visiting families are welcome to bring their children to the program while they get acquainted with us. Visitor forms can be used for the first three visits, after which full registration with this form is required.

You do not have to be a member to register your child. Registration is ongoing throughout the year. There is no registration fee. However, please consider the value of these programs when you determine your yearly pledge to NSUC.

Our religious education program is dependent upon volunteer participation to operate. The most effective way to instill your values in your child is by your actions. You can model the importance of religious education for your child by your own participation and volunteering your time. Furthermore, when more people volunteer, the load is lighter for everyone, and there is more of a sense of teamwork among members. This year, we expect to have a greater number of short-term/guest teaching opportunities alongside our traditional longer-term teaching commitments.

This year we are asking for details of your child or youth's musical abilities and interests. We will share this information with the music staff, so they can invite your child to participate in the various worship services and events throughout the year.

CHILDREN'S RELIGIOUS EDUCATION REGISTRATION
Enter information for babies through 8th graders here. Go to Youth Group section to enter youth in grades 9-12.
Which of your children will be participating in Junior Choir?
Junior Choir meets on Sunday mornings at 11:20am-12:00pm and sings during all-ages worship services. Children in kindergarten through 8th grade are welcome. Junior Choir is part of the larger music program at NSUC, and this information will be shared with Timothy Mah, Junior Choir Director.
If your child sings or plays an instrument and would be interested in providing special music for worship services occasionally, please list their name and talent here.
This information will be shared with Rev. Lucas Hergert and Stephen Blackwelder, Music Director.
List anyone who will be participating in Our Whole Lives for 7th - 8th Graders this year.
OWL is a comprehensive sexuality education program created by the Unitarian Universalist Association and the United Church of Church to support young people in developing relationship and decision-making skills, understanding their bodies, and understanding social and cultural challenges. The meeting times/dates of this class are still being determined, but has often been held before worship from Sept. to April.

YOUTH GROUP REGISTRATION
Youth Group is for young people in grades 9-12 and is offered on Sunday mornings during worship when there is not an all-ages service. Youth are expected to attend worship service through the Message for All Ages before going to the youth room.

Completing the youth email and cell phone fields gives permission to the church's representatives to use the youth's email address and cell phone number for relevant communications, in addition to the parents' contact information.

You are allowing your child(ren) to participate in activities that may be photographed or videotaped in a building that regularly hosts public events. NSUC posts printed images on bulletin boards, maintains a website and social media presence, and disseminates its news and events electronically. We cannot guarantee that your child's image will not appear publicly. However, we will ask that names not be used and promise a reasonable effort towards all children's safety. *
NSUC is a spiritual community that takes seriously its responsibility to provide a safe and nurturing environment for everyone, with an emphasis on our children and youth. By registering your children, you agree to abide by the guidelines in the Congregational Safety Policy and make sure your children/youth are supervised when at the church. *
